The absolute basophil count (ABC) measures the actual number of basophils per mm³ of blood. Basophils are normally present at very low levels (<100 cells/mm³) and their absolute quantification is most useful in the context of haematological disorders.

Persistent absolute basophilia (>100 cells/mm³) is a significant finding that warrants haematological investigation, as it is a hallmark feature of chronic myeloid leukaemia (CML) and other myeloproliferative neoplasms. In the context of CML treatment monitoring, declining basophilia signals treatment response.
