Urinary casts are cylindrical structures formed in the kidney tubules, composed primarily of Tamm-Horsfall protein (uromodulin) and can contain cells, crystals, or hemoglobin. Occasional hyaline casts (0–2 per lpf) may be seen in healthy individuals, but higher numbers or other cast types can indicate underlying kidney stress or disease.
