Understanding HOMA-IR: Normal vs Optimal Ranges
Also known as: HOMA IR, Homeostatic Model Assessment of Insulin Resistance
?What is HOMA-IR?
HOMA-IR (Homeostatic Model Assessment of Insulin Resistance) is a calculated index that estimates insulin resistance from simultaneous fasting glucose and fasting insulin measurements. The formula is: (fasting insulin × fasting glucose) / 405 (for mg/dL units). A higher HOMA-IR indicates greater insulin resistance.
!Why It Matters
HOMA-IR provides a practical, non-invasive surrogate for the gold-standard insulin clamp test. It is widely used in research and increasingly in clinical practice to identify insulin resistance before it progresses to prediabetes or diabetes. A HOMA-IR above 2.0–2.5 suggests clinically significant insulin resistance; values above 3.0 indicate severe resistance.
Reference Ranges
| Range Type | Min | Max | Unit | Note |
|---|---|---|---|---|
| Lab Normal | — | 2.5 | (calculated) | Standard lab reference range |
| Optimal | 0 | 1.5 | (calculated) | Evidence-based optimal range for health |
| Longevity Target | 0 | 1.5 | (calculated) | Per longevity medicine research (Attia et al.) |
Lab normal ranges may vary between laboratories. Optimal and longevity targets are based on research literature and should be interpreted with your physician.
Symptoms of Imbalance
- Fatigue, especially after carbohydrate-rich meals
- Cravings for sweets and carbohydrates
- Difficulty losing weight
- Abdominal obesity
How to Improve Your Levels
- 1All interventions that improve insulin sensitivity lower HOMA-IR
- 2Exercise is the most potent intervention — both aerobic and resistance training
- 3Low-carbohydrate diet significantly reduces HOMA-IR within weeks
- 4Weight loss of even 5–10% body weight meaningfully improves insulin resistance
When to Test
HOMA-IR is calculated from fasting insulin and fasting glucose, both of which must be ordered. It is not a separate blood draw. Retest every 3–6 months when actively managing insulin resistance.
