Understanding IL-6: Normal vs Optimal Ranges

Also known as: il-6, interleukin 6, il6, interleukin-6

Inflammatory MarkersUnit: pg/mL

?What is IL-6?

Interleukin-6 (IL-6) is a multifunctional cytokine produced by immune cells, adipose tissue, and muscle. It acts as both a pro-inflammatory and anti-inflammatory signal depending on context. IL-6 is a key driver of the acute-phase response and is elevated in infections, autoimmune diseases, and chronic inflammatory states.

!Why It Matters

Chronically elevated IL-6 is associated with sarcopenia, cardiovascular disease, cancer cachexia, depression, and metabolic syndrome. In COVID-19, IL-6 'cytokine storms' drove life-threatening lung inflammation. IL-6 is a therapeutic target — tocilizumab (anti-IL-6 receptor antibody) is used in rheumatoid arthritis and severe COVID-19.

Reference Ranges

Range TypeMinMaxUnitNote
Lab Normal7pg/mLStandard lab reference range
Optimal7pg/mLEvidence-based optimal range for health
Longevity Target7pg/mLPer longevity medicine research (Attia et al.)

Lab normal ranges may vary between laboratories. Optimal and longevity targets are based on research literature and should be interpreted with your physician.

Symptoms of Imbalance

  • Elevated IL-6: fatigue, muscle wasting, fever, night sweats
  • Chronic elevation: weight loss, depression, reduced physical performance

How to Improve Your Levels

  • 1Regular moderate exercise transiently raises IL-6 (beneficial metabolic signal) but reduces baseline chronic levels
  • 2Anti-inflammatory diet (Mediterranean, plant-forward)
  • 3Weight loss — visceral fat is a major IL-6 source
  • 4Adequate sleep reduces inflammatory cytokines
  • 5Treat underlying autoimmune or infectious conditions

When to Test

In suspected autoimmune conditions, severe infections, monitoring biologic therapy, or advanced metabolic panels.
