Understanding Immature Granule Percentage (IG%): Normal vs Optimal Ranges
Also known as: immature granulocytes, left shift, bands and stabs
?What is Immature Granule Percentage (IG%)?
Immature granule percentage (IG%) measures the proportion of immature neutrophils and other granulocytes in the peripheral blood, expressed as a percentage of total white blood cells. These immature forms are typically released from bone marrow during acute infection, inflammation, or stress to the immune system. An elevated IG% is often referred to as a "left shift," indicating accelerated release of young immune cells.
!Why It Matters
IG% can be a sensitive early marker of bacterial infection, sepsis, or acute inflammatory stress before symptoms fully develop or standard white blood cell counts rise. Elevated levels may suggest the body is mounting an active immune response, while persistently low or absent immature granules could indicate bone marrow dysfunction or chronic immune suppression.
Reference Ranges
| Range Type | Min | Max | Unit | Note |
|---|---|---|---|---|
| Lab Normal | 0 | 5 | % | Standard lab reference range |
| Optimal | 0 | 3 | % | Evidence-based optimal range for health |
| Longevity Target | 0 | 3 | % | Per longevity medicine research (Attia et al.) |
Lab normal ranges may vary between laboratories. Optimal and longevity targets are based on research literature and should be interpreted with your physician.
Symptoms of Imbalance
- Fever or signs of infection
- Fatigue and malaise
- Unexplained inflammation or swelling
- Rapid onset of systemic symptoms
- Severe stress or critical illness
How to Improve Your Levels
- 1Identify and treat underlying infection promptly with appropriate antimicrobial therapy if warranted
- 2Support immune function through adequate sleep, rest, and stress management during acute illness
- 3Ensure adequate protein and micronutrient intake, particularly vitamin C, zinc, and iron
- 4Maintain hydration to support bone marrow and immune cell production
- 5Avoid immunosuppressive behaviours such as smoking and excessive alcohol
- 6Monitor closely with serial blood work to track resolution of the acute process
When to Test
When infection or acute inflammation is suspected; as part of sepsis screening or monitoring in critically ill patients; during fever of unknown origin workup; and to assess bone marrow recovery in patients with haematologic disease or after chemotherapy.
