?What is LP-IR Score?

The Lipoprotein Insulin Resistance (LP-IR) Score is an NMR-derived composite score calculated from six lipoprotein variables: large VLDL particle concentration, large HDL particle concentration, and medium LDL particle concentration (inversely related), plus small LDL, small HDL, and VLDL size. It quantifies insulin resistance at the lipoprotein level before fasting glucose or HbA1c become abnormal — making it an early metabolic warning signal.

LP-IR detects insulin resistance with sensitivity comparable to HOMA-IR but without requiring insulin measurement. Studies show LP-IR predicts progression to type 2 diabetes, cardiovascular events, and metabolic syndrome. Because it captures the atherogenic dyslipidaemia pattern (elevated VLDL, small dense LDL, low large HDL) specifically driven by insulin resistance, it provides mechanistic risk information beyond standard lipids.
