Understanding Omega-3 Index: Normal vs Optimal Ranges

Also known as: Omega 3 Index

Fatty Acid RatiosUnit: %

?What is Omega-3 Index?

The Omega-3 Index measures the percentage of EPA (eicosapentaenoic acid) and DHA (docosahexaenoic acid) in red blood cell membranes. Unlike serum omega-3 fatty acids (which reflect recent intake), the red cell index reflects average omega-3 status over the past 4 months — making it a reliable long-term biomarker.

!Why It Matters

Research by Dr. William Harris and colleagues established that an Omega-3 Index above 8% is associated with the lowest risk of sudden cardiac death, while below 4% carries significantly higher risk. DHA is the dominant structural fat in the brain — low omega-3 index is associated with cognitive decline, depression, and poor neurodevelopment. The ratio of omega-6 to omega-3 in Western and modern Indian diets may exceed 20:1, when optimal is around 4:1.

Reference Ranges

Range TypeMinMaxUnitNote
Lab Normal416%Standard lab reference range
Optimal816%Evidence-based optimal range for health
Longevity Target816%Per longevity medicine research (Attia et al.)

Lab normal ranges may vary between laboratories. Optimal and longevity targets are based on research literature and should be interpreted with your physician.

Symptoms of Imbalance

  • Low omega-3 index: depression, anxiety, brain fog
  • Dry skin, dry eyes
  • Joint stiffness and inflammation
  • Poor cardiovascular risk profile

How to Improve Your Levels

  • 1Eat fatty fish 2–3 times per week (salmon, sardines, mackerel, anchovies)
  • 2Supplement with high-quality fish oil (2–4 g/day of EPA+DHA) — the most reliable way to raise the index
  • 3Plant-based ALA (from flaxseed, chia, walnuts) converts poorly to EPA/DHA in most people
  • 4Algae-based DHA is an effective fish-free option
  • 5Reduce omega-6 intake: replace refined vegetable oils (sunflower, soybean) with olive oil

When to Test

Test with a red blood cell fatty acid profile. Retest after 4–6 months of consistent supplementation. Not widely available in India — consider specialty labs.
