Understanding Thyroid Panel: Normal vs Optimal Ranges

Also known as: Thyroid Function Test, TFT, Thyroid Blood Test, TSH Panel

Panel Overview

?What is a Thyroid Panel?

A Thyroid Panel measures hormones produced by the thyroid gland and pituitary to assess thyroid function. It diagnoses hypothyroidism (underactive), hyperthyroidism (overactive), and monitors treatment for thyroid disorders.

What Does It Measure?

  • TSH (Thyroid Stimulating Hormone) — pituitary signal to thyroid
  • Free T4 (Thyroxine) — main hormone produced by thyroid
  • Free T3 (Triiodothyronine) — active thyroid hormone
  • Thyroid Antibodies (TPO, TG) — autoimmune thyroid disease markers

!Why It Matters

Thyroid disorders affect 1 in 8 women and 1 in 20 men. Hypothyroidism causes fatigue, weight gain, depression, and high cholesterol. Hyperthyroidism causes anxiety, weight loss, palpitations, and osteoporosis. Early detection and treatment prevents complications like heart disease and infertility.

Component Breakdown

How to Prepare

  • No fasting required for most thyroid tests
  • Take biotin supplements? Stop 2-3 days before test (interferes with results)
  • Test at the same time of day (TSH has diurnal variation — highest in morning)
  • If on thyroid medication, take it after the blood draw

What Do Results Mean?

High TSH + low Free T4 = hypothyroidism (underactive). Low TSH + high Free T4/T3 = hyperthyroidism (overactive). High TSH + normal Free T4 = subclinical hypothyroidism. Positive TPO antibodies indicate Hashimoto's (autoimmune). Normal TSH with symptoms may need Free T3 and Reverse T3 testing.

Next Steps

Hypothyroidism: start levothyroxine (T4), retest in 6-8 weeks. Hyperthyroidism: endocrinology referral for methimazole, radioactive iodine, or surgery. Subclinical: monitor or treat based on symptoms and TPO status. Positive antibodies: monitor annually even if TSH normal.
