?What is PSA (Total)?

Prostate-specific antigen (PSA) is a serine protease enzyme produced almost exclusively by prostate epithelial cells. Total PSA measures both free and bound PSA in serum. It is the most widely used prostate cancer screening biomarker and is also elevated in benign prostatic hyperplasia (BPH) and prostatitis.

!Why It Matters

Elevated PSA prompts further evaluation for prostate cancer, typically with MRI and/or biopsy. However, PSA lacks specificity — 75% of biopsies triggered by elevated PSA do not find cancer. PSA velocity (rate of rise), PSA density, free/total PSA ratio, and newer tests (PHI, 4Kscore) improve cancer detection specificity. PSA is indispensable for monitoring treatment response after prostate cancer diagnosis.
