?What is Total T4?

Total T4 (thyroxine) measures all T4 in blood — bound to thyroid-binding globulin (TBG), albumin, and transthyretin, plus the small free fraction. T4 is the main secretory product of the thyroid gland and acts as a prohormone, requiring conversion to the more active T3 in peripheral tissues.

!Why It Matters

Total T4 has largely been superseded by Free T4 for thyroid assessment because Total T4 is affected by TBG levels (elevated in pregnancy and OCP use; decreased in liver disease, nephrotic syndrome). However, Total T4 is still used in some settings and provides complementary information when protein binding is expected to be altered.
