Understanding Vitamin E (α-Tocopherol): Normal vs Optimal Ranges

Also known as: alpha-tocopherol, tocopherol, serum vitamin e

VitaminsUnit: mg/L

?What is Vitamin E (α-Tocopherol)?

Vitamin E (alpha-tocopherol) is the principal fat-soluble antioxidant in human cell membranes, protecting polyunsaturated fatty acids from lipid peroxidation. It also modulates immune function, platelet aggregation, and gene expression. Serum alpha-tocopherol is the standard measurement; results should be interpreted adjusted for total lipid levels since vitamin E is transported in lipoproteins.

!Why It Matters

Vitamin E deficiency causes neurological damage (peripheral neuropathy, spinocerebellar ataxia) and haemolytic anaemia — particularly in premature infants and those with fat malabsorption syndromes (cystic fibrosis, abetalipoproteinaemia). High-dose supplementation (>400 IU/day) has been associated with increased all-cause mortality in some meta-analyses and may increase bleeding risk in those on anticoagulants.

Reference Ranges

Range TypeMinMaxUnitNote
Lab Normal5.517mg/LStandard lab reference range
Optimal5.517mg/LEvidence-based optimal range for health
Longevity Target5.517mg/LPer longevity medicine research (Attia et al.)

Lab normal ranges may vary between laboratories. Optimal and longevity targets are based on research literature and should be interpreted with your physician.

Symptoms of Imbalance

  • Deficiency: peripheral neuropathy, muscle weakness, ataxia, impaired vision
  • Toxicity: bleeding, impaired platelet function, vitamin K antagonism

How to Improve Your Levels

  • 1Dietary sources: sunflower seeds (highest), almonds, hazelnuts, wheat germ oil, olive oil
  • 2Supplementation at physiological doses (15 mg/day) is safe; avoid megadoses
  • 3Treat underlying fat malabsorption to improve vitamin E absorption
  • 4Natural mixed tocopherols (food form) are preferable to synthetic dl-alpha-tocopherol

When to Test

Fat malabsorption syndromes; premature infants; suspected neuropathy from deficiency; monitoring high-dose supplementation.
