Understanding Apolipoprotein A-1: Normal vs Optimal Ranges
Also known as: apolipoprotein a1, apolipoprotein a-i, apo a1, apo a-1, apoa1, apolipoprotein a-1
?What is Apolipoprotein A-1?
Apolipoprotein A-1 (ApoA-1) is the primary protein component of high-density lipoprotein (HDL) particles. It plays a central role in reverse cholesterol transport — the process that moves cholesterol from peripheral tissues back to the liver for elimination. ApoA-1 is considered a more accurate measure of HDL function than HDL cholesterol alone.
!Why It Matters
Low ApoA-1 is independently associated with increased cardiovascular risk, even when HDL cholesterol appears normal. It reflects the number and functionality of HDL particles available for reverse cholesterol transport. The ApoB/ApoA-1 ratio is one of the strongest predictors of future cardiovascular events in large epidemiological studies.
Reference Ranges
| Range Type | Min | Max | Unit | Note |
|---|---|---|---|---|
| Lab Normal | 120 | 176 | mg/dL | Standard lab reference range |
| Optimal | 120 | 176 | mg/dL | Evidence-based optimal range for health |
| Longevity Target | 120 | 176 | mg/dL | Per longevity medicine research (Attia et al.) |
Lab normal ranges may vary between laboratories. Optimal and longevity targets are based on research literature and should be interpreted with your physician.
Symptoms of Imbalance
- Typically asymptomatic — low ApoA-1 is a lab finding
- May co-occur with signs of metabolic syndrome: abdominal obesity, insulin resistance
How to Improve Your Levels
- 1Regular aerobic exercise is the most effective way to raise ApoA-1
- 2Avoid trans fats and replace saturated fats with unsaturated fats
- 3Moderate alcohol intake may raise HDL/ApoA-1 — though benefits must be weighed against risks
- 4Quit smoking, which depresses ApoA-1
- 5Niacin and fibrates can raise ApoA-1 pharmacologically
When to Test
Part of advanced cardiovascular risk panels; especially useful in those with intermediate 10-year CV risk.
