Understanding Apolipoprotein B: Normal vs Optimal Ranges
Also known as: ApoB, Apo B, ApoB100
?What is Apolipoprotein B?
Apolipoprotein B (ApoB) is a protein that forms the structural backbone of all atherogenic lipoprotein particles — LDL, VLDL, IDL, and Lp(a). Because each of these particles contains exactly one ApoB molecule, measuring ApoB directly counts the number of potentially artery-clogging particles, rather than just their cholesterol content.
!Why It Matters
ApoB is now considered the most accurate single measure of cardiovascular risk from lipids. Landmark research including the INTERHEART study found that ApoB/ApoA1 ratio predicted heart attack risk in populations worldwide better than LDL-C. Dr. Peter Attia and other longevity physicians recommend targeting ApoB below 80 mg/dL for low cardiovascular risk, and below 60 mg/dL for those with established disease or high risk.
Reference Ranges
| Range Type | Min | Max | Unit | Note |
|---|---|---|---|---|
| Lab Normal | — | 130 | mg/dL | Standard lab reference range |
| Optimal | 0 | 80 | mg/dL | Evidence-based optimal range for health |
| Longevity Target | 0 | 80 | mg/dL | Per longevity medicine research (Attia et al.) |
Lab normal ranges may vary between laboratories. Optimal and longevity targets are based on research literature and should be interpreted with your physician.
Symptoms of Imbalance
- No direct symptoms from elevated ApoB
How to Improve Your Levels
- 1Same interventions as for LDL — statins, PCSK9 inhibitors, and dietary changes all lower ApoB
- 2Reducing saturated fat and increasing soluble fibre are most effective dietary strategies
- 3Weight loss reduces the number of VLDL particles produced by the liver
When to Test
ApoB is not included in standard lipid panels but is available as an add-on test. Recommended for anyone with borderline risk, diabetes, metabolic syndrome, or discordance between LDL-C and cardiovascular risk. Also valuable when triglycerides are high (which inflates calculated LDL).
