?What is Immunoglobulin A?

Immunoglobulin A (IgA) is the most abundant antibody in mucosal secretions (gut, respiratory tract, genitourinary tract) and the second most abundant in serum. Secretory IgA is the primary first-line immune defence at mucosal surfaces, preventing pathogen adhesion and translocation. Serum total IgA reflects both mucosal immune activity and systemic B-cell immunoglobulin production.

!Why It Matters

Total IgA is essential context for interpreting gut permeability tests: IgA-deficient individuals (~1 in 500 in general population, more common in coeliac disease) will have false-negative IgA-based tests (tTG-IgA, DGP-IgA, candida IgA). Elevated total IgA may indicate IgA nephropathy (Berger's disease), coeliac disease, or systemic autoimmunity. Low IgA predisposes to recurrent respiratory and gastrointestinal infections.
