Understanding Niacin: Normal vs Optimal Ranges
Also known as: nicotinic acid (niacin)
?What is Niacin?
Nicotinic acid (niacin, vitamin B3 acid form) is one of three main vitamin B3 forms. It has distinct pharmacological effects at high doses (1–3 g/day) compared to physiological nutrition-level intake: raising HDL-C by 20–35%, lowering triglycerides by 20–50%, and slightly reducing LDL. Nicotinic acid is converted to NAD+ via a separate pathway (Preiss-Handler pathway) from nicotinamide. The hallmark side effect of pharmacological doses is prostaglandin-mediated cutaneous flushing.
!Why It Matters
Nicotinic acid (measured in plasma or urine) reflects dietary B3 intake and metabolic conversion. At therapeutic doses, nicotinic acid has potent lipid-modifying effects and was historically a cornerstone of cardiovascular prevention. Although cardiovascular outcome trials (AIM-HIGH, HPS2-THRIVE) showed mixed results with statin combination, nicotinic acid remains relevant for refractory dyslipidaemia and familial hypertriglyceridaemia. Monitoring plasma nicotinic acid ensures adequacy and avoids deficiency.
Reference Ranges
| Range Type | Min | Max | Unit | Note |
|---|---|---|---|---|
| Lab Normal | 10 | 80 | µmol/L | Standard lab reference range |
Lab normal ranges may vary between laboratories. Optimal and longevity targets are based on research literature and should be interpreted with your physician.
Symptoms of Imbalance
- Deficiency: pellagra (dermatitis, diarrhoea, dementia), glossitis
- Fatigue, depression, and memory impairment
- Pharmacological doses: flushing, pruritus, hepatotoxicity at high doses
How to Improve Your Levels
- 1Dietary sources: meat, fish, poultry, peanuts, mushrooms, whole grains
- 2Tryptophan-containing foods also contribute (60:1 conversion ratio)
- 3Pharmacological niacin for dyslipidaemia — taken with aspirin to minimise flushing
- 4Extended-release niacin reduces flushing compared to immediate-release
When to Test
Pellagra or B3 deficiency investigation; as part of B vitamin profile; baseline before pharmacological niacin therapy.
