?What is Niacin?

Nicotinic acid (niacin, vitamin B3 acid form) is one of three main vitamin B3 forms. It has distinct pharmacological effects at high doses (1–3 g/day) compared to physiological nutrition-level intake: raising HDL-C by 20–35%, lowering triglycerides by 20–50%, and slightly reducing LDL. Nicotinic acid is converted to NAD+ via a separate pathway (Preiss-Handler pathway) from nicotinamide. The hallmark side effect of pharmacological doses is prostaglandin-mediated cutaneous flushing.

!Why It Matters

Nicotinic acid (measured in plasma or urine) reflects dietary B3 intake and metabolic conversion. At therapeutic doses, nicotinic acid has potent lipid-modifying effects and was historically a cornerstone of cardiovascular prevention. Although cardiovascular outcome trials (AIM-HIGH, HPS2-THRIVE) showed mixed results with statin combination, nicotinic acid remains relevant for refractory dyslipidaemia and familial hypertriglyceridaemia. Monitoring plasma nicotinic acid ensures adequacy and avoids deficiency.
