Pathological casts are cylindrical protein structures formed in the kidney tubules and found in urine during microscopic urinalysis. Their presence in urine may suggest underlying kidney stress, dehydration, or systemic disease. Casts are classified by their composition (cellular, granular, hyaline) and presence typically warrants further clinical evaluation.
