Understanding Total Testosterone: Normal vs Optimal Ranges
Also known as: Testosterone Level (Total), Testosterone Total, Serum Testosterone
?What is Total Testosterone?
Total testosterone measures all testosterone in the blood — both free (biologically active) and bound to proteins (SHBG and albumin). It is the primary male sex hormone, produced mainly in the testes, responsible for muscle mass, bone density, libido, mood, cognitive function, and red blood cell production.
!Why It Matters
Low testosterone (hypogonadism) is increasingly common in men of all ages and is associated with reduced muscle mass, increased fat (especially visceral), fatigue, depression, poor sleep, reduced libido, and higher cardiovascular risk. Even 'normal' testosterone in the lower half of the reference range may be associated with suboptimal wellbeing. Testosterone naturally declines ~1–2% per year after age 30.
Reference Ranges
| Range Type | Min | Max | Unit | Note |
|---|---|---|---|---|
| Lab Normal | 300 | 1000 | ng/dL | Standard lab reference range |
| Optimal | 500 | 900 | ng/dL | Evidence-based optimal range for health |
| Longevity Target | 500 | 900 | ng/dL | Per longevity medicine research (Attia et al.) |
Lab normal ranges may vary between laboratories. Optimal and longevity targets are based on research literature and should be interpreted with your physician.
Symptoms of Imbalance
- Fatigue and reduced energy
- Decreased muscle mass and strength
- Increased body fat, especially around the abdomen
- Reduced libido and erectile dysfunction
- Depression, irritability, reduced motivation
- Poor sleep and concentration
How to Improve Your Levels
- 1Resistance training — the most potent natural stimulus for testosterone production
- 2Prioritise sleep (7–9 hours); testosterone is primarily produced during deep sleep
- 3Maintain healthy body weight — obesity strongly suppresses testosterone
- 4Optimise vitamin D and zinc — cofactors for testosterone synthesis
- 5Manage stress and cortisol — cortisol suppresses testosterone
- 6Avoid alcohol excess — hepatotoxicity and direct testicular suppression
- 7Consider ashwagandha — has modest clinical evidence for raising testosterone
When to Test
Test fasting, preferably in the morning (8–10 AM) when testosterone is highest. Always test total testosterone alongside SHBG and free testosterone for complete picture. Repeat any abnormal result to confirm.
