Understanding ALT (SGPT): Normal vs Optimal Ranges

Also known as: SGPT, Alanine Aminotransferase, ALT

LiverUnit: U/L

?What is ALT (SGPT)?

Alanine aminotransferase (ALT), also called SGPT (Serum Glutamic Pyruvic Transaminase), is an enzyme found predominantly in the liver. It is the most liver-specific of the standard liver enzymes. When liver cells are damaged or inflamed, ALT is released into the bloodstream, making it a sensitive indicator of liver health.

!Why It Matters

ALT is the primary marker for detecting hepatocellular (liver cell) damage. Research by Dr. Robert Prati and others suggests that even 'normal' ALT values may be too lenient — truly healthy individuals often have ALT below 25 U/L in men and 20 U/L in women. Persistently elevated ALT, even mildly, is associated with increased risk of liver fibrosis, cardiovascular disease, and metabolic dysfunction.

Reference Ranges

Range TypeMinMaxUnitNote
Optimal725U/LEvidence-based optimal range for health
Longevity Target725U/LPer longevity medicine research (Attia et al.)

Lab normal ranges may vary between laboratories. Optimal and longevity targets are based on research literature and should be interpreted with your physician.

Symptoms of Imbalance

  • Mildly elevated ALT is usually asymptomatic
  • Significant elevation: fatigue, nausea, right upper abdominal discomfort
  • Jaundice and dark urine in severe hepatitis

How to Improve Your Levels

  • 1Fatty liver is the most common cause — lose weight, reduce refined carbohydrates
  • 2Avoid hepatotoxic substances: alcohol, high-dose paracetamol, some herbal supplements
  • 3Regular exercise directly reduces liver fat and ALT
  • 4Vitamin E (800 IU/day) is effective for non-alcoholic fatty liver under medical supervision
  • 5GLP-1 agonists show promise for reducing ALT in fatty liver disease
  • 6Avoid high-dose niacin and some over-the-counter supplements without medical guidance

When to Test

Included in standard liver function tests. Should be retested 3–6 months after intervention. Consult a doctor if ALT is above 3 times the upper limit of normal on repeat testing.
