?What is Total Bilirubin?

Bilirubin is a yellow pigment produced during the normal breakdown of red blood cells. The liver processes bilirubin and excretes it in bile. Total bilirubin is the sum of direct (conjugated) and indirect (unconjugated) bilirubin. Elevated bilirubin causes jaundice — the yellowing of skin and eyes.

!Why It Matters

Elevated total bilirubin can result from increased red blood cell destruction (haemolysis), impaired liver processing, or obstruction of bile flow. Mildly elevated indirect bilirubin in isolation, with normal liver enzymes, often indicates Gilbert's syndrome — a benign hereditary condition affecting about 3–8% of the population. Bilirubin itself is a powerful antioxidant and mildly elevated levels (within normal range) may actually be cardiovascularly protective.

Symptoms of Imbalance

• Jaundice: yellowing of skin and whites of eyes
• Dark urine (tea-coloured) when bilirubin is conjugated and excreted in urine
• Pale or clay-coloured stools in bile duct obstruction
• Itching (pruritus) in cholestatic liver disease
