Understanding AST (SGOT): Normal vs Optimal Ranges
Also known as: SGOT, Aspartate Aminotransferase, AST
?What is AST (SGOT)?
Aspartate aminotransferase (AST), also known as SGOT (Serum Glutamic Oxaloacetic Transaminase), is an enzyme found mainly in the liver, heart muscle, and skeletal muscle. It is released into the bloodstream when these cells are damaged. AST is commonly measured as part of a liver function test panel.
!Why It Matters
Elevated AST can indicate liver inflammation from fatty liver disease, hepatitis, alcohol, medications, or other causes. Because AST is also present in heart and skeletal muscle, it is less liver-specific than ALT — a significantly elevated AST with normal or mildly elevated ALT may suggest heart or muscle injury. The AST/ALT ratio (de Ritis ratio) provides diagnostic information about the type of liver condition.
Reference Ranges
| Range Type | Min | Max | Unit | Note |
|---|---|---|---|---|
| Optimal | 10 | 30 | U/L | Evidence-based optimal range for health |
| Longevity Target | 10 | 30 | U/L | Per longevity medicine research (Attia et al.) |
Lab normal ranges may vary between laboratories. Optimal and longevity targets are based on research literature and should be interpreted with your physician.
Symptoms of Imbalance
- Mild AST elevation often causes no symptoms
- Jaundice (yellowing of skin/eyes) in significant liver disease
- Fatigue, nausea, loss of appetite
- Abdominal discomfort in the right upper quadrant
- Dark urine, pale stools
How to Improve Your Levels
- 1Eliminate alcohol or reduce significantly
- 2Identify and stop hepatotoxic medications or supplements
- 3Achieve a healthy weight — fatty liver is a common cause of elevated liver enzymes
- 4Reduce refined carbohydrates and fructose intake
- 5Coffee consumption (2–3 cups/day) is consistently associated with lower liver enzyme levels
- 6Increase physical activity — exercise reduces liver fat
When to Test
Part of standard liver function tests. Recheck 4–12 weeks after making dietary changes. Significant elevations (>3× upper normal limit) warrant immediate medical evaluation.
