Beta cell function (%B) is calculated from the HOMA-B (Homeostatic Model Assessment of Beta Cell Function) equation using fasting glucose and fasting insulin. It estimates the percentage of normal beta cell insulin secretory capacity. A value of 100% represents the mean function of a healthy non-diabetic reference population. %B >100% suggests compensatory hyperinsulinaemia; %B <100% indicates beta cell secretory impairment.

Beta cell loss in type 2 diabetes begins well before diagnosis and is largely irreversible once significant. HOMA-%B tracking enables detection of declining beta cell reserve years before HbA1c rises to diagnostic thresholds.
