Understanding Insulin Sensitivity %S: Normal vs Optimal Ranges

Also known as: insulin sensitivity (%s)

MetabolicUnit: %

?What is Insulin Sensitivity %S?

Insulin Sensitivity %S is calculated from the HOMA-S (Homeostatic Model Assessment of Insulin Sensitivity) equation using fasting glucose and fasting insulin. It estimates sensitivity to insulin relative to a healthy non-diabetic reference population (100%). Values below 100% indicate insulin resistance; values below 50% represent significant insulin resistance requiring clinical attention. HOMA-S is the reciprocal of HOMA-IR.

!Why It Matters

Insulin resistance — reflected by low %S — is the root metabolic defect underlying type 2 diabetes, metabolic syndrome, PCOS, NAFLD, and cardiovascular disease. Detecting impaired insulin sensitivity early provides the most powerful opportunity to intervene before irreversible organ damage occurs. HOMA-%S tracks the response to lifestyle, dietary, and pharmacological insulin-sensitising interventions over time.

Reference Ranges

Range TypeMinMaxUnitNote
Lab Normal80120%Standard lab reference range
Optimal80120%Evidence-based optimal range for health
Longevity Target80120%Per longevity medicine research (Attia et al.)

Lab normal ranges may vary between laboratories. Optimal and longevity targets are based on research literature and should be interpreted with your physician.

Symptoms of Imbalance

  • Abdominal obesity and difficulty losing visceral fat
  • Post-meal energy crashes and carbohydrate cravings
  • Acanthosis nigricans (darkened skin in skin folds)
  • Elevated fasting triglycerides and low HDL
  • PCOS in women; erectile dysfunction in men

How to Improve Your Levels

  • 1Low-glycaemic or low-carbohydrate diet — reduces insulin demand and improves sensitivity within weeks
  • 2Exercise: both resistance training and aerobic exercise independently improve insulin sensitivity
  • 3Lose 5–10% body weight — most impactful single intervention
  • 4Berberine (500 mg 3×/day) — shown to match metformin in insulin sensitisation
  • 5Optimise sleep: even one week of sleep restriction worsens insulin sensitivity by 25%
  • 6Reduce stress — cortisol and adrenaline directly oppose insulin action

When to Test

Insulin resistance evaluation; prediabetes or diabetes risk; PCOS workup; NAFLD investigation; cardiovascular risk in metabolically unhealthy individuals; monitoring metabolic treatment.
