The Neutrophil-to-Lymphocyte Ratio (NLR) is calculated by dividing the absolute neutrophil count by the absolute lymphocyte count from a standard CBC. It is a simple, widely available marker of immune balance and systemic inflammatory stress. Neutrophils represent the innate inflammatory response, while lymphocytes represent adaptive immune function; the ratio between them reflects the body's inflammatory versus anti-inflammatory immune balance.

NLR is one of the strongest readily available prognostic markers in medicine. Elevated NLR (>3) independently predicts worse outcomes in cancer, cardiovascular disease, sepsis, COVID-19, and major surgery. It also predicts all-cause mortality in the general population. An NLR above 3 in an otherwise healthy individual suggests significant underlying inflammatory or immunological stress worthy of investigation.
