?What is Vitamin B12?

Vitamin B12 (cobalamin) is an essential water-soluble vitamin involved in DNA synthesis, red blood cell formation, myelin sheath maintenance (protecting nerves), and one-carbon metabolism. It is found almost exclusively in animal products. Absorption requires intrinsic factor, a protein produced in the stomach.

!Why It Matters

B12 deficiency causes megaloblastic anaemia, peripheral neuropathy, subacute combined degeneration of the spinal cord, and cognitive decline. Many of these effects are irreversible if deficiency is prolonged. Serum B12 can be misleading — some people have 'normal' total B12 but functional deficiency; methylmalonic acid (MMA) and homocysteine are better functional markers.

  • 3Intramuscular injections if absorption is impaired (pernicious anaemia, gastric surgery)
  • 4Check for pernicious anaemia with anti-intrinsic factor antibodies if unexplained deficiency
  • 5Avoid antacids and metformin long-term without B12 monitoring

When to Test

Annually for all strict vegetarians and vegans. Also test if there is unexplained fatigue, neuropathy, anaemia with high MCV, or memory problems. Test homocysteine and MMA for functional B12 status.
